Legend holds that Anthony retreated to the desert to live in ascetic solitude, where he was tormented by the Devil. The Devil sent demons to beat him, promised him worldly power, and tempted him with lust to break his spirit.

Bosch does not paint a simple saint; he paints the psychological chaos of the temptation. Anthony appears calm and meditative amidst a whirlwind of chaos, representing the steadfastness of faith against the madness of the world.

1. The Left Panel: The Flight and Fall

The narrative begins in the sky. St. Anthony is shown being borne aloft by a swarm of bizarre demons, who beat and torment him. Below, an exhausted Anthony is helped across a bridge by two monks and a layman (believed by some to be a self-portrait of Bosch). The landscape is weird and unsettling, featuring a figure crouched on all fours forming a hill—a classic Boschian hallucination.

2. The Center Panel: The Ultimate Temptation

The centerpiece of the work is a riot of symbolism. St. Anthony kneels in the center, looking out at the viewer, his hand raised in blessing. He is surrounded by a "Black Mass" or mock banquet. To his left, a city burns (representing the fire of St. Anthony). The scene is populated by Bosch’s signature hybrids: fish-boats, walking heads, and armored monsters. It is a depiction of a world gone mad, where the saint remains the only anchor of sanity.

3. The Right Panel: The Contemplation

Here, the Temptation takes a subtler form. St. Anthony is shown reading, while a naked woman (the Devil in disguise) appears from a hollow tree trunk, offering the temptation of lust.
